This role is essential for providing specialized wound care consultations in both office and patient home settings. The nurse will manage 15-20 consults per day, with 70% of the responsibilities conducted in-office and 30% requiring travel to patients' homes in Manhattan, Brooklyn, the Bronx, and Queens. Key Responsibilities: Evaluate wounds and develop tailored treatment plans. Provide quality wound care consultations in compliance with healthcare standards. Collaborate with multidisciplinary teams to enhance patient outcomes. Requirements: Certification: Wound Care Nurse or Wound Ostomy Care Nurse Certification (mandatory). License: Current New York State Registered Nurse License. Education: Bachelor's Degree in Nursing (required). Experience: Minimum 2 years of experience in a Certified Home Health Agency or post-acute care facility. 2+ years of Public/Community Health and 2+ years of Medical/Surgical experience. Familiarity with PI applications and facilitation of PI programs is a plus.
