Qualifications: Education, Knowledge, Skills and Abilities Required: Completion of an accredited LPN program Licensed Practical Nurse AHA Basic Health Care Life Support HCP Certification Excellent written and verbal communication skills. Responsibilities: The LPN collaborates with the multidisciplinary team to provide patient and family-centered nursing care to diverse patient populations. Assist the unit Leadership Team to maintain safe and quality clinical standards, while supporting efforts to improve the patient experience. Responsible for ensuring assigned patients receive all medications, treatments, and nursing care as directed. Completes all required nursing documentation. Follow a plan of care for assigned patients. Consistently, accurately, and independently complete all tasks associated with the nursing care assignment. Continuously identify and address all patient and family needs. Demonstrate effective communication with patients, families, and visitors delivered with sensitivity and respect. Assist with the admission and discharge process under the supervision of the RN. Administer medications and treatments as needed. Evaluate and document patient responses to interventions with escalation to the RN as appropriate. Report any changes in the patient's clinical condition to the primary RN and/or Charge Nurse. Respond to all emergency situations on the unit. Communicate and collaborate with the Nursing team to solve complex issues related to patient care. Participate and represent the Nursing team on committees, as requested by the Nurse Leader. Maintains professional competence through timely completion of all required education and training. Other duties and/or projects as assigned.
