Pay rate range between $23-$25 hourlySchedule:Saturday 7:00 AM -3:00 PM, Sunday 11:00 AM -7:00 PM, Tuesday and Wednesday 3:00 PM-11:00 PMWork as part of a clinical multi-disciplinary team.Provides medication assistance to consumers residing in a Community Living setting.Department Goals:To provide timely Recovery Based Clinical and Non- Clinical supports services to tegrate quality assurance/clinical excellence processes to provide the safe delivery of quality Behavioral Health care within reasonable financial resource limits.Develop and maintain sound clinical programs that promote community and independent living.To enhance SBH's community image, community relations, and consumer confidence. Review and analyze aggregated data in order to help enhance the overall performance of the department Job Duties: Monitor and educate clients on known health conditions such as diabetes, high blood pressure, and obesity.Assist consumers in self-administration of medications in accordance with provider ordersReview the effects and side effects of medication adherenceProvide monitoring of physical/medical status and notify appropriate medical personnel with concernsAccepts medication orders from physician via telephone and medication records in accordance with State regulations, indications and resources.Works with prescribers in the monitoring of medications and changing of medication pletes charting and record keeping per SBH/ADHS standards.Participate in Clinical case staffing.Facilitate coordination with the consumer's clinical team and PCP, and any specialists as needed.Assists in SBH chart audit process.Qualifications: Must be a LPN, currently licensed in the States of Arizona. Must have post-degree experience of two years in the area of behavioral health and/or healthcare nursing.
