Job Overview: Schedule: Monday-Friday, 10am-6pm (Flexibility to work occasional late night) Salary : $52,000 non-exempt + full benefits package Location: Lower East Side (Hybrid) Organizational Overview: Henry Street Settlement opens doors of opportunity to enrich lives and expand human progress for Lower East Side residents and other New Yorkers through social services, arts, and health care programs. Building on its 125-year legacy of innovation and effective programming, Henry Street offers more than 50 programs and is one of New York City's largest social service agencies. Third Street Supportive Housing Residence is a permanent supportive housing program inaugurated in 2007. We are part of the agency's Transitional & Supportive Housing Division and provide on-site social services for 52 tenants. At the residence, the wellness and safety of the individual is deeply connected to the wellness and safety of our community. Qualifications: Bachelor's degree in social work or another health/social services discipline required Minimum of two (2) years' experience delivering case management services to individuals with severe and persistent mental illness, history of homelessness and substance use dependence Demonstrated experience and knowledge of New York State supportive housing benefits, entitlements and services available for special needs populations Strong individual and group facilitation skills Bilingual (English/Spanish) preferred Good interpersonal and team building skills Must be able to work occasional late shift and maintain flexibility in scheduling Responsibilities: Provide case coordination to assist residents in accessing comprehensive bio-psycho-social care and advocacy in maintaining public benefits Maintain up-to-date, accurate individual case records on each assigned tenant and develop measurable and objective service plans that maximize tenants' rehabilitative abilities in accordance with regulatory guidelines Obtain and develop resource information for tenants in all aspects of their care; disseminate this information to them in a manner that is clear and understandable Occasionally travel with tenants to regularly scheduled appointments, emergency visits to medical facilities, social service agencies, government offices, or other locations associated with the treatment or assistance of the tenant Immediately report serious incidents, allegations, or sensitive situations to supervisors Provide direct services including individual case management and group activities Network, refer, and coordinate care with other collateral providers ex. APS, primary care physician, psychiatrist, employer, substance use program and home health aide provider Provide crisis intervention as needed Other related duties as assigned by supervisor Essential Physical Requirements Fully vaccinated against COVID-19 Ability to work onsite in the Lower East Side Must be able to ascend/descend 6 flights of stairs
