Job Title: Clinical Nurse Supervisor Location: New York, New York Employment Type: Full-Time Department: Integrated Care Overview: The Nurse Supervisor is responsible for conducting thorough evaluations for prospective residents (both internal and external) as well as re-evaluations of current residents. This role involves assessing residents' needs across various domains, developing personalized care plans, and collaborating with interdisciplinary teams to ensure residents receive the highest quality of support. Key Responsibilities: Conduct comprehensive assessments for new prospective residents and re-evaluations for current residents using ICH-approved tools, Yardi EHR, and NY DOH forms. Complete evaluations across multiple settings, including homes, skilled nursing facilities, and hospitals, to gather a holistic view of residents' needs. Assess physical health, cognitive abilities, social needs, and activities of daily living (ADL) skills to determine appropriate levels of care and support. Develop individualized service plans in collaboration with the community's Interdisciplinary Care Team (ICT) based on assessment findings, ensuring all recommended services and supports align with residents' needs. Communicate with healthcare professionals, residents, and their families to review evaluation outcomes, service plans, and available resources. Regularly monitor and reevaluate residents to adjust care plans as necessary, ensuring continued appropriateness and effectiveness. Accurately document assessment findings and service plans, maintaining all resident records in accordance with regulatory standards. Serve as a knowledgeable resource for residents and families, providing guidance on available services and resources. Participate actively in ICT and multidisciplinary meetings to discuss resident care plans and evaluation results. Stay informed on best practices, regulatory standards, and emerging trends in assisted living evaluations and care planning. Review prior nursing assessments and medical notes to identify and implement necessary updates to service plans. Gather essential clinical information from various healthcare sources to accurately determine the level of care required by prospective residents and confirm the community's capacity to meet those needs. What We are Looking For: Must be a Licensed Practical Nurse (LPN) or Registered Nurse (RN), licensed to practice in the state of New York. Ability to travel around downstate New York, including the 5 boroughs, Long Island, Westchester and parts of New Jersey.
