Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Job Description1.Accepts full responsibility and accountability of unit report by making walking rounds with incoming/outgoing staff.3.Checks for absent and late employees and monitors the sign in and sign out sheets for accuracy in documentation of attendance.4.Ensures staff compliance with daily morning/evening briefs/huddles during shift change.5.Ensures staff compliance with patient-centered intershift reporting that includes the patient in handoff communications, as well as updating the bedside patient whiteboard staff utilization of SBAR for all handoff patient ' staff to perform functional assignments based on individual competency and patient skill mix.8.Delegates assignments based on staff competence and priority of patient care utilizing the patient classification system and follow up.9.Conducts patient rounds on the unit daily with Service Excellence initiatives as a top priority.10.Follow-up with all work requests to ensure needs of the unit are met timely.11.Supervises medication treatment and diagnostic procedures administered or performed by the staff.12.Performs daily environmental unit rounds utilizing Joint Commission compliance resource tools, and to assure compliance with infection Control and Safety Policies.13.Ensures unit operations are in compliance with all Joint Commission and Regulatory Agency requirements.14.Facilitates a culture of continuous quality and performance improvement using the Breakthrough A3 model of performance improvement for the with Nursing Quality to ensure staff compliance with unit, organizational and regulatory agency policy and procedures.16.Role models and supports organizational strategic priorities in an enthusiastic and positive manner.17.Ensures staff compliance with patient-centered hourly rounding utilizing the 6 Ps.18.Submits all reports and assignments by instructed patient care by collaborating with the multidisciplinary municates effectively with the patient/family, medical, nursing and other staff.21.Escalates changes in patient's medical condition to the Interdisciplinary team.22.Maintains standards of care by ensuring that an individual plan of care for each patient is developed and implemented.23.Supervises and assists the staff in patient assessments, documentation, implementation and evaluation of Nursing Care Plan in the Electronic Medical Record.24.Ensure staff compliance with the implementation of Patient/Family teaching upon admission, and ensures continuity of patient/family education throughout supervision of all staff.26.Ensures unit supplies are adequate to meet all patient care pletes performance evaluation for assigned staff, and promotes staff development and continuing as clinical resource person for staff, and assumes patient care responsibilities as the need pletes time schedule for staffing and submits timely to the ADON/ADN for approval.30.Participates in the development and in the implementation of unit orientation to all new employees.31.Counsels staff in collaboration with the ADON/ADN, and makes recommendations for further corrective action.32.Keeps anecdotal records of significant occurrences on the unit of staff behavior and performance issues.33.Attends and participates in committee meetings as assigned.34.Conducts monthly staff meetings to communicate all unit, organizational and regulatory agency updates and or changes to policies/procedures.35.Actively participates in pilot studies to improve unit and organizational clinical competencies by attending continuing education programs.37.Responsible for reporting to the ADON/ADN any situation or problem on the pletes end of shift report and provides a handoff to the ADN coming on duty.39.Participates and supports Team Building, Problem Solving, and Breakthrough Initiatives to enhance unit and organizational the Breakthrough A3 model for all performance improvement projects on the unit.41.Identifies and engages staff to promote safety, quality, and performance improvement on the unit.42.Performs all related duties as assigned.Minimum Qualifications1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA); and3.
