Each year, MercyFirst serves more than 3,000 children, teenagers and families overcome enormous obstacles, re-imagine their futures and develop their full potential.PROGRAM BACKGROUND:To meet the needs of New York City's ever-changing foster care population. Our children have often experienced significant levels of trauma, poverty and violence. As they grow and mature, their individual needs change. Our assessments, health counseling, and selected treatments change to meet their new needs. Our children's physical and mental health as well as their strengths must be supported so they can reach their full potential.POSITION SUMMARY:Under the supervision of the Nursing Supervisor, the LPN will be responsible for providing best practice nursing care to our clients. Our clients are a mix of infants, toddlers, school age children and teens and are male and female. LPNs will work with birth families, foster parents, and other members of each child's treatment team. REQUIRED QUALIFICATIONS: Current Licensed Practical Nurse in New York State, current CPR certification and completion of required courses in Child Abuse and Infection Control. Must have and maintain a valid Driver's License with a satisfactory driving record. RESPONSIBILITIES: Conduct physical health assessments for infants, children, and adolescents including HIV Risk Assessments, General First Aid, Triage Activities for "sick" calls. Routine Screening for Substance Abuse, Developmental Health, Mental health, Physical Body Checks for Signs of AbuseRoutine management and training regarding chronic conditions (Asthma, Diabetes)Take vital signs, heights, and weights; document observations and activities Provide immunizations and document sameProvide first aid and emergency care as neededExecute medical orders (tests, tasks, hospitalizations, etc.) as ordered by physician/nurse practitioner/psychiatristObtain certain lab specimensAdminister hearing and vision testsTraining and Health Education related to Sexual Reproductive HealthMedication Administration and Management ReconciliationEnsure required documentation for all case actions (medical notes) are completed within established timeframes. Full-time 40 hours/week.
