Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Job DescriptionPURPOSE OF POSITIONThis position is being created to have a Nurse Practitioner on a 9:00AM to 5:00PM schedule for Behavioral Health Inpatient Units to ensure compliance with the 7 day rounds in Psychiatry.Areas Of ResponsibilitiesINPATIENT BEHAVIORAL HEALTHSummary Of Duties And ResponsibilitiesUnder a collaborative agreement with an attending Psychiatrist and with the latitude for independent initiative and judgment, the Psychiatric Nurse Practitioner will perform the following functions: Provides professionalclinical services to patients in lnpatient Psychiatry to assess and addressthe need for psychiatric andor substance abuse treatment, psychiatric medication(s),screening and referral for medical care, and follow-up as appropriate. Performs a comprehensive psychiatric evaluation that includes assessing current psychiatricsymptoms and past psychiatric history, the psychosocial and physical health status, evaluationof mental status, current and past history of violence, suicidal or self-harm behavior, substanceuse, level of functioning, health behaviors, trauma, and social and developmental history. Completes Initial Psychiatric Assessment, Suicide Risk Assessment, AIMS, and appropriaterating scales on the first treatment day with patient. Reviews the medical record if the patient has had prior hospitalizations, Contacts the patient's outpatient psychiatrist (or clinician)to exchange clinical information ifpatient has been treated outside of hospital.Documents patient's clinical and treatment status daily for first five working days and aminimum of three times a week thereafter. There must also be a note when there is asignificant change in the patient's condition or treatment. Documents any psychiatric, behavioral, substance abuse and active medical problems on theProblem List and Comprehensive Treatment Plan within three days of admission. Develops a differential diagnosis derived from the collection and synthesis of assessmentdata. Consults with other members of the interdisciplinary team to determine, evaluate, and plantreatment goalsobjectives, Updates and completes the Treatment Plan Review weekly after the completion of theComprehensive Treatment Plan andor whenever there is a significant change in the patient'scondition or treatment. Records patient's status and progress of care rendered, and plan for future care andtherapeutic interventions appropriate to the patient's needs. Counsels and guides patients, their families, andor significant others relating to physicalhealth, psychiatric treatment, and progress, Request appropriate laboratory tests within established protocols, and interprets findings ofrelevant diagnostic and laboratory tests. Prescribes and makes adjustments on medication within established protocols, and followsuch medication(s) interventions as appropriate' Manages psychiatric emergencies by determining the level of risk and initiating andcoordinating effective emergency care. Serves as team leader in psychiatric crisis situations. Ensures that patient and staff debriefings are conducted immediately following Code Greys orother behavioral crises, leading the debriefings if possible' Ensures that a patient BehavioralSafety Plan is in place for all patients, and that teammembers are aware of and consider patient preferences in responding to a crisis. Consults with the physician, as appropriate, for evaluation of findings of the psychiatric andhealth screening in order to make prospective decisions about treatment. Ensures the timely and appropriate management of a patient's medical problems by the unit'sinternist.
