A FQHC in New York City is actively seeking a Licensed Practical Nurse (LPN) to join their Family Medicine team in Manhattan.About the Opportunity:Start Date: ASAPAssignment Length: OngoingSchedule: Monday to Friday (1 Saturday per month)Hours: 8am to 6pm (Mon & Wed); 8am to 5pm (Tues, Thurs & Fri); 8am to 4pm (Sat)Call: NonePatient Population: Medically UnderservedPatient Volume: 20 to 24 per dayEMR: eClinicalWorks (ECW)Responsibilities:The LPN will:Assess, plan, implement, and evaluate patient care plansMonitor, record and report symptoms and changes in patients' conditionRecord patients' medical information and vital signsMonitor inventory of medications and immunizationsOrder medications and immunizations as neededEvaluate and monitor all aspects of patient care, including diet and physical activityPrepare patients for, and assist with, examinations and treatmentsEducate individuals, families and other groups on topics such as their health conditions, disease prevention, proper medication use, and develop health improvement programsEnsure that stock of clinical supplies is maintainedProvide healthcare, first aid, immunizations and other nursing servicesEnter patient's data into the medical recordsAdminister drugs, immunizations, medical devices or other forms of treatmentPerform other duties, as neededQualifications:Graduate of an accredited Practical Nursing programNYS Licensed Practical Nurse (LPN)ACLS and CPR certificationsOutpatient Primary Care backgroundDesired Skills:Bachelor's Degree in NursingExperience in an Ambulatory Care, Medical-Surgical, Primary Care, andor Women's HealthExperience working with indigent or medically underserved populationsBilingual Spanish
