Family Nurse Practitioner Virtual Care

Position Details:Job Summary:•Evaluate primary care, acute, chronically ill, and transitional care patients, providing healthcare education, counseling, and disposition planning for patients aged 18 months and above.•Provide patient counseling on pregnancy prevention, STI prevention/safer sex practices, contraceptive care counseling, and medication management.•Educate patients on health maintenance and respond to patient care inquiries.•Document all patient care within an EHR according to MinuteClinic policies and procedures.•Coordinate care with internal and external colleagues, ensuring the highest standard of care.•Work effectively within a patient care team, including fellow Providers, Collaborative Physicians, para-professionals, Pharmacists, and other healthcare team members.Key Responsibilities:•Deliver excellent customer service.•Increase patient engagement and satisfaction through feedback integration.•Create a warm and welcoming environment focused on the patient.•Tailor communication style to influence quality outcomes and meet patient needs.•Collaborate with pharmacy and front store colleagues to provide a complete patient experience.•Maintain patient confidentiality in accordance with PHI and HIPAA standards.Required Qualifications:•Must be willing/available to work every other weekend.•Must be willing/available to work two major holidays and one minor holiday per year.•Minimum of two years of medically-relevant experience.•Basic Life Support (BLS) certification.•High-speed/broadband internet connectivity with a download speed of at least 25 Mbps and upload speed of 3 Mbps.Certifications:•Basic Life Support (BLS) : Basic Life Support
