Job DescriptionUPMC Your Care is hiring a full-time paramedic to provide urgent and episodic care to patients in their homes and communities.The paramedic will work under the supervision of a physician and collaborate with other healthcare professionals on the UPMC Your Care interdisciplinary team to deliver high-quality, patient-centered, and cost-effective care.Adheres to system standardization, policies and procedures, and other established practices to ensure prompt efficient response, safe operations, and quality patient care. Meets all deadlines for certifications, clinical competencies, or educational requirements. Ensures safe operation of patient-laden stretcher employing appropriate stretcher discipline at all times. Maintains safe loading, unloading, adjustment of the head of the stretcher, and stretcher movement during transports. We currently see patients 7 days a week, and shifts are variable. Some weekends and evenings are required to support the program goal of emergency department avoidance. However, the schedule commonly accommodates our team's goals and responsibilities outside of work. Additional education is provided during orientation, so the role is ideal for those looking to diversify their expertise and augment their autonomy for community-based practice. Demonstrates the ability to establish a rapport with patients, caregivers, and healthcare providers; including the ability to create and sustain a positive relationship; regardless of circumstances, environment, or patient presentation.Employee Health Maintains fitness for duty by adhering to all employee health requirements and required medical screenings. Reports injuries and completes the appropriate documentation to a supervisor on time, no matter how minor.CLINICAL Performs thorough hands-on member assessments and history-taking and clinical exams, utilizing diagnostic equipment including ECG, point-of-care laboratory testing, and other advanced adjuncts to investigate symptoms and complaints. Assesses members for geriatric syndromes such as falls, functional decline, sleep disorders, delirium, dementia, bladderbowel problems, polypharmacy, and weight loss. Understands care for the older adult and palliative care interventions. Assess, treat, and monitor members with acute or chronic conditions, such as wounds, infections, pain, diabetes, etc., using appropriate clinical skills and protocols. Conducts home safety assessments Provides emergency care when indicated by a life-threatening condition. Provide advanced clinical care to ill or injured patients in adherence to established protocols and standards of care Accesses online medical command and communicates clinical findings verbally andor in writing, providing a clear impression of the patient's condition. Utilizes telemedicine technology to facilitate high-fidelity remote examination by a physician in adjunct to the paramedic's assessment. Confirms and performs interventions and treatments based on physician orders. Administers medications with attention to safety and accuracy. Facilitates delivery of prescription medications from stock as ordered. Ensures that patients understand all instructions to remain safely home following treatment and receive appropriate follow-up care. Adheres to a high level of professionalism in documentation, including thorough notation of appropriate assessment, interventions, and other pertinent patient information on time.Collects and promptly processes demographic and medical necessity information. Participates constructively in the peer-review-driven quality improvement program.OPERATIONS Attends and participates in face-to-face case conferences, team meetings, and other work-related meetings. Reports for scheduled shifts on time and well-rested. Responds to all requests safely and efficiently, adhering to system standards, policies and procedures, and other established practices. Participates in projects and initiatives to continue the advancement of this growing team. Participates constructively in the peer-review-driven quality improvement program. Presents or contributes to complex case reviews by the interdisciplinary team summarizing clinical and social history, current medications, geriatric syndromes, healthcare resource utilization, and case management interventions. Maintains operational readiness by completing required equipment and medication checks and logs on an established schedule. Document and report member status, interventions, outcomes, and follow-up plans in a timely and accurate manner, using the electronic health record system and other tools. Coordinate and communicate with the UPMC Your Care interdisciplinary team, the supervising provider, and other healthcare professionals to ensure continuity and quality of care. Maintain and update clinical knowledge and skills through continuing education, training, and certification. Adhere to all applicable laws, regulations, policies, and procedures related to Medicare, home health, and paramedic practice. Demonstrate professionalism, compassion, and respect for members, families, and colleagues. Meets all deadlines for certifications, clinical competencies, or educational requirements.SAFETY Safely operates a company vehicle, avoiding distractions while driving. Supports safe vehicle operation by coworkers, providing navigation and communication support during response. Ensures the safety of patients and others in and around the patient's home. Utilizes appropriate channels to report safety concerns and uses tools available to enhance a safe work environment.? Maintains fitness for duty by adhering to all employee health requirements and required medical screenings. Reports and documents injuries, no matter how minor, to a supervisor in a timely manner.High School Diploma or EquivalentThree years of clinical experience in prehospital or hospital care. Must be eligible to receive medical command authorization by the Medical Director, have reliable transportation, and be willing to travel to any residence or venue of care within the defined service area. Must be able to successfully complete the prescribed orientation program, including classroom, clinical rotations, educational competencies, and on the job training.Must have the ability to work productively and effectively within a complex environment, and handle multiplechanging priorities, and specialized equipment.
