Grace and MercyHome Care Services, LLC(Caring Is Our Pride!)OCCUPATIONAL THERAPIST JOB DESCRIPTIONREPORTS TO: Director of Patient ServicesDEPARTMENT: TherapyTIME / STATUS: Fulltime / Per Diem/ Per VisitBENEFITS: Based on Schedule RESPONSIBILITIES AND DUTIES:1. Develop and participate in health promotion programs, group activities, or discussions to promote patient health, facilitate social adjustment, alleviate stress, and prevent physical or mental disability2. Plan, organize, and conduct occupational therapy to patients to help rehabilitate those impaired because of illness, injury, or psychological or developmental problems.3. Consult with the rehabilitation team to select activity programs and coordinate occupational therapy with other therapeutic activities4. Test and evaluate patients' physical and mental abilities and analyze medical data to determine realistic rehabilitation goals for them5. Select activities that will help patients learn work and life-management skills within limits of their mental and physical capabilities6. Plan and implement programs and social activities to help patients learn work and school skills and adjust to handicaps7. Recommend changes in patients' work or living environments consistent with their needs and capabilities8. Help patients improve decision-making, abstract reasoning, memory, sequencing, coordination, and perceptual skills using computer programs9. Lay out materials such as puzzles, scissors, and eating utensils for use in therapy and clean and repair these tools after therapy sessions10. Provide training and supervision in therapy techniques and objectives for Home Health Aides (HHAs) to assist patients when needed11. Recommend special supplies and equipment, such as splints, braces, and computer-aided adaptive equipment12. Evaluate patients' progress and prepare reports that detail progress. Complete and maintain necessary records.13. Provide patients with assistance in locating and holding jobs.14. Assist Director of Patient Services to formulate company policies and procedures for physical therapy.15. Participate in departmental and general meetings as needed*Duties are not restricted to the above-mentioned; they may be amended to meet company needs.QUALIFICATIONS:1. Must possess A New York state Occupational License 2. Must be a graduate of a program in occupational therapy approved by the Council on Medical Education of the American Medical Association in collaboration with the American Occupational Therapy Association, or 3. A graduate of a curriculum in occupational therapy which is recognized by the World Federation of Occupational Therapists and is eligible for registration with the American Occupational Therapy Association, and4. Must be registered with the American Occupational Therapy skills:2. Must be able to relate appropriately to, communicate effectively with, and develop trusting relationships with diverse patient population and with staff. 3. Must be able to meet or exceed the Service Excellence Standards of the New York State Department of Health 4. Must be able and willing to work on an as-needed basis in order to meet operational needs of the agency. 5. Being bi-lingual in Spanish/English will be a plus6. Preferably two years of experience with one year home care experience Minimum Salary: 30000 Maximum Salary: 50000 Salary Unit: Yearly
