Work Shifts 8:30 A.M - 4:30 P.M Job Description PURPOSE OF POSITION The Licensed Social Worker LVL III is an LCSW who works independently as well as in collaboration with transdisciplinary teams as a primary therapist who identifies psychosocial issues and concerns of patients and their families and provide clinical services to meet their needs. The Mental Health Continuum is a collaboration of various agencies, including the Department of Education. Through this collaboration, the Social Worker will support the behavioral health needs of students in the South Bronx. They function as a therapist in the Child Outpatient Mental Health Clinic and a school-based clinic servicing child and adolescents with severe mental health conditions and co-morbid medical and substance use disorders. The goal is to provide meaningful and stable connections between school staff, who know their students and families' best, and mental health service providers committed to serving their local communities; this will allow Lincoln to more rapidly see students in need of urgent evaluation, assessment, treatment and referrals for additional support. The SW LVL III provides clinical supervision and support to the SW LVL II staff. SUMMARY OF DUTIES AND RESPONSIBILITIES The duties include functioning as a primary therapist for subset of patients treated in the Child and Adolescent Outpatient Clinic and School Based Clinics, facilitates group therapy, conducting initial psychiatric assessment, documenting biopsychosocial assessments, formulation and disposition, developing comprehensive treatment plans, writing discharge summaries and other related paperwork. The therapist is responsible for facilitating supportive, psychoeducational and insight-oriented psychotherapeutic interventions, providing crisis interventions patients, engaging with and providing support to families, and engaging them in recovery. The candidate is responsible for identifying and collaborating with existing support systems, facilitating community referrals for patients as needed collaborating with interdisciplinary team to develop most appropriate case dispositions, while documenting all interventions within the required regulatory timeframes. In collaboration with select schools, the Social Worker will provide timely mental health clinic appointments, urgent care, and tele-video visits to the students while in school or at home.
