Work Shifts 9:00 A.M - 5:00 P.M Job Description PURPOSE OF POSITION: To provide telephonic follow up outreach to patients discharged from Inpatient Psychiatry. AREAS OF RESPONSIBILITIES: Patients discharged from the Adult Inpatient Psychiatry Units. SUMMARY OF DUTIES AND RESPONSIBILITIES: 1. Makes calls to patients following discharge to determine if they have reached their discharge destination safely and have attended their scheduled aftercare appointment. 2. Explore with the patient and/or family/significant others the barriers/resistances that are preventing the patient from adhering to the prescribed treatment. The Koskinas social worker will remain involved for 30 days following discharge to work with patients and their families to connect to treatment. 2. Reschedules aftercare appointments if patient did not attend initial scheduled appointment. 3. Provides concrete services as deemed necessary 4. Maintains records and statistics. 5. Meets with patients and staff when indicated. 6. Completes data entry in EPIC Computer Program. 7. Completes relevant reports. 8. Attends Department of Psychiatry and Social Work meetings. 9. Reports to Social Worker level V Minimum Qualifications For Assignment to Level II 1. Valid license as an LMSW issued by the NYSED; or 2. Valid license as an LCSW issued by the NYSED.
