Job Description - Residential Services, Direct Care Service Title: Registered Nurse/Licensed Practitioner Nurse Responsible to: Sr. Regional Program Director General Description of Duties and Responsibilities: Provides clients with nursing services in accordance with their needs. Participates, as appropriate, in the development, review, and update of individual program plans (ISPs); the development, with a physician, of a medical care plan when and if required by a physician; assists in the review of clients' health status; refers to a physician to address client health problems as necessary; renders other nursing care as prescribed by a physician or as identified by client needs, within scope of practice; and implements, with other direct care staff, appropriate protective and preventive health measures. Participates in the training/orientation of new nursing staff. Specific Duties: Provide routine nursing care. Administer medications and/or train staff as prescribed by a physician by: ensuring correct dosage is given and accurately charted and maintaining an accurate count to ensure security. Schedule quarterly, annual and follow-up medical appointments based on OPD regulation. Participate, as appropriate, in the development of the nursing aspects of ISP for assigned clients by providing information on physical and mental health as observed through nursing new admission or existing participant assessment. Implement nursing aspects of the ISP. Provide the CEO or designated person with a monthly report on individuals seen care plan (needs, updates and/or changes). Train participants and staff as needed in appropriate health and hygiene methods. Provide remediation training as needed. Conduct HRST nursing assessments. As needed, taking vital signs and observing behavior as needed. Participate in admission and discharge decision-making based on medical information. Review hospital discharge summary and provide staff training and/or information when necessary. Maintain accurate medical records by documenting nursing care provided in Therap medical health records. Document medical concerns in Therap medical health records and monthly reports. Notify physician and/or Regional Program Director of medical concerns. Assist in the control of communicable disease and infections, including the instruction of other personnel in methods of infection control. Assist in the training of direct care staff in detecting signs and symptoms of illness or dysfunction, first aid for accidents or illness, and basic skills required to meet the health needs of clients. Develop internal medical protocols in collaboration with PCP and/or other medical professions to ensure best care and support. Ensure that the nursing plan of care for active treatment is carried out. Ensure that client's rights are observed at all times. Participate in training/orientation of new nursing staff. Comply with and enforce facility safety procedures. Comply with and enforce all facility administrative policies. Completes medication records i.e., Medication Account Records, Physician Order Sheets, Nursing Tracking Forms etc. as per established schedule. Works with and coordinates care with pharmacy, physicians, and other health care providers. Participates in the clinical treatment team as scheduled. Renews physician's orders and review for accuracy. Provides 1:1 nursing supervision when needed. Assesses/treats any health problems as needed: check blood sugars, administers insulin, checks vital signs, provides care for wounds, administer suppositories/enemas. Reads reports and other lab results. Assists on-call staff with questions related to individuals' health and/or medication concerns. Monitors employee medication errors and takes necessary action if assigned. Other duties as assigned. QUALIFICATIONS AND REQUIREMENTS: Graduation from an approved school of practical nursing and three years of experience as an RN/LPN in an acute care, long-term care, or MH/MR facility, to include at least two years of experience as an RN/LPN. Must be a Licensed RN/LPN. General knowledge in the theory and practice of practical nursing as applied to the Intellectual development disabled (IDD) and MH. Working knowledge of basic medical nursing procedures. A basic understanding of the development and implementation of treatment plans for the care of IDD/MH clients. Skill in writing sufficient to prepare and maintain accurate medical records. Ability to establish and maintain good working relationships with staff and clients. Ability to follow physicians' directions. Skill in communicating with persons from diverse backgrounds. Ability to perform assigned duties under limited supervision, following-up as required. In addition, the incumbent must possess the knowledge, skills, and abilities required at the RN/LPN level. Ability to direct the work of lower-level staff. Current license to practice as a practical nurse RN/LPN in PA. Current CPR/First Aid Certification. Physically able to perform required duties; ability to lift and carry 70 lbs., and ability to successfully pass lifting screening and training. Must complete direct care training; and must complete orientation and all other required training satisfactorily in order to continue employment. RN/LPN must complete the recommended training program as required. Experience in working with the Intellectually Developmentally Disabled preferred. Must have a valid driver's license and driving record acceptable to the insurance carrier. Must be 21 years of age or older. Must clear ACT 33, 34 and 73 (criminal).
