Job Description The Social Worker Level V provides consultation and ongoing supervision to staff and respective units. As a member of interdisciplinary teams, the supervisor provides comprehensive psychosocial assessments and interventions which support the continuing care needs of patients and their families. Tasks may include: Providing clinical and administrative supervision and orientation to staff assigned to their unit and other professional disciplines. Ensuring that all patients are provided with timely and complete Social Work services. Demonstrating effectiveness in planning, organizing, and managing the ongoing operation of their units Provide biopsychosocial assessment per referral and assessment policy, plan and carry out appropriate interventions Complete required documentation in timely fashion, according to assigned area requirements and in professional manner (see documentation policy) Provide ongoing support and psychotherapeutic counseling to patients, families/significant others as well as concrete services at frequency determined by respective assigned area As indicated, contact patient's families, collaterals, significant others as well as community and medical providers and include/share this information in assessment, documentation and during multidisciplinary meetings. Collaborate with family, collaterals and providers in terms of treatment, referrals and / or aftercare planning Escorting patients into the community and making home visits as needed Attend rounds, and meetings with multidisciplinary teams Advocating and negotiating with related social and community agencies to secure services for patients Provide caring, nonjudgmental, ethical services to patient Participating in Performance Improvement Projects Compliance with all department, corporate and hospital regulations, policies and procedures and conformity to established dress guidelines, time and leave requirements as an essential worker, and need to wear identification badge while on duty Incorporate universal screening of patients to identify if they are a victim of abuse. Follow hospital policies on interventions regarding child abuse, intimate partner violence, elder abuse, sexual assault and human trafficking, as well as New York State protocols for mandated reporting of suspected child abuse or maltreatment. Participation in regular supervision and ad-hoe supervision /escalation as necessary to seek guidance from appropriate personal Perform work related duties with understanding that as a hospital employee, one is categorized as an essential worker, which includes coming to assignment under a variety of circumstances such as inclement weather, pandemic and other extreme circumstances. Other tasks that may be assigned Minimum Qualifications Direct patient care and supervise social work staff in cancer center, general adult primary care, and subspecialty ambulatory care clinics. Provide social work assessment, concrete services, and referrals as needed. Serve as behavioral health resource, provide crisis and supportive counseling as needed, and system navigation to aide in pts' connection to care. Involves program development, with attention to patient engagement strategies, streamlining referral workflows, and working with complex patient population in the outpatient setting. Work hours are Monday-Friday, regular business hours.
