POSITION SUMMARY: Esperanza Health Center currently seeks a board certified Family Medicine or Med/Peds Physician to provide Christ-centered primary care to the predominantly Latino North Philadelphia community. Spanish functionality (or willingness to learn) is required. Providers have received loan repayment via National Health Service Corps, the Pennsylvania Loan Repayment Program, or Project MedSend. DUTIES AND RESPONSIBILITIES Essentials At work or staff meeting on time. Communicates with medical director if more than 10 minutes late. Completes documentation of clinic visits within 24 hours of the patient visit. If rounding at hospital, completes hospital billing promptly. Checks and responds promptly to urgent care-related calls and messages. Maintains credentials including license, DEA, hospital privileges, and continuing education. Demonstrates lifestyle consistent with biblical standards Takes ownership of and attains personal patient volume targets. Clinical Team Function Arrives at patient sessions on time in AM and PM, prepared with appropriate items. Remains in hub with focus on clinical care (not returning to office, not sidetracked by other non-emergent tasks.) Oversees an effective team huddle, reviewing patient and schedule issues, preparing for the expected. Uses walkie-talkies well (doesn't leave room unnecessarily, communicates needs to MA and clerical assistant, keeps walkie-talkie on person at all times). Assigns support staff to work on needed items (lab orders, shots, appts, referrals, etc) before and during visit, rather than saving all until the end, to decrease pt waiting time. Doesn't do things support staff can do (referrals, phone calls, getting samples, etc), unless support staff unavailable. Communicates well with support staff, giving them needed details and instructions, so that they know provider expectations. Works to accommodate walk-in and squeeze-in pts. Takes share of ownership of all EHC clinical activities, including directing management of unscheduled patient needs. Adjusts pace of seeing pts to clinical demand. Empowers clerical assistant to control pt flow and make effective decisions regarding walk-ins/squeeze-ins. Schedules brief follow-ups (blood pressure checks, PPD checks, etc) as double-booked appts instead of sending to triage, so that these are anticipated by team and handled efficiently. Plans routine lab work for follow-up of chronic problems 1-2 wks before follow-up appt. Teaches support staff in order to expand/improve their role (pt educational tasks, medication instructions, diabetes topics, etc. Encourages support staff to take a role in spiritual care of pts. Empowers patients by encouraging them to be the primary agent in seeking wellness, encouraging them to make own appts and seek solutions to improve own health. Sets good boundaries with patients (plans visit agenda, defers issues to subsequent appts when necessary, polite and compassionate yet firm). Prescribes controlled medications according to established guidelines, utilizing appropriate accountability measures. Completes charting and referrals as sees each patient as much as possible. Does each day's paperwork on that day. Works efficiently to decrease patient waiting time and finishes AM and PM patient care sessions on time. Encouraging, positive, and constructive in interactions with team members. Gives feedback and contributes ideas to team. Is approachable and teachable, receiving constructive criticism well. Keeps work area neat and organized. Documents in medical record all patient interactions while on call. Follows up on messages and refill requests within 2 working days. Completes forms within 5 working days. Regularly reviews lab and note review bins. Clinical Skills Demonstrates appropriate clinical judgment in clinical assessment and treatment, following established clinical care guidelines. Demonstrates appropriate knowledge of specialty areas. Plans appropriate follow-up for chronic problems, abnormal tests, and no shows. Requests consultation with specialists when appropriate. Effectively communicates with and educates patients. Responds quickly to urgent/emergent needs. Communicates appropriately with outside health professionals. Maintains confidentiality regarding patient information. Addresses established EHC priority QA measures. Provides appropriate preventive care and utilizes Health Maintenance table for documentation of care. Keeps chronic problems up to date on problem list. Keeps current medication list up to date, performing medication reconciliations with transitions of care. Practices cost-effective medicine with regard to tests and treatments recommended. Performs procedures with appropriate consent, documentation, skill, care, follow-up, and with minimal complications. Tracks all higher risk labs and referrals to assure proper follow-up Reports and completes adverse event form related to any errors or near misses, even if no harm comes to the patient. If any complications are experienced, communicates with and follows-up with patient, completes adverse event form, and informs medical director and liability agency as appropriate. Practices according to biblical ethical worldview, ascribing sacred value to all human life from the time of conception. Accordingly does not refer for or perform elective pregnancy terminations, and chooses family planning methods which minimize the possibility of fertilization and/or destruction of fertilized ovum. Spiritual and Holistic Care Assesses psychological needs of patients and makes referrals to counseling and psychiatry as appropriate. Assesses social needs of patients and refers to social services as appropriate. Provides effective spiritual intervention with patients, sharing hope and salvation through Jesus Christ, sharing helpful Biblical instruction and scriptures, praying for pts, and connecting/referring to Christian churches as appropriate and according to the leading of the Holy Spirit, motivated always by love. Avoids taking advantage of status and privilege as practitioner as opportunity for spiritual coercion or manipulation. Gives spiritually uninterested or antagonistic pts greatest respect and appropriate space, continuing to provide exemplary care. Serves all pts in a Christ-like manner, with respect, humility, love, and self-control. Interpersonal Relationships Communicates directly and in a timely fashion with co-workers to resolve conflict/disagreements Avoids negative comments in hearing of others; refrains from discussing any information critical of anyone in the hearing of uninvolved persons. Expresses enthusiasm, contributes to morale of other staff. Additional Responsibilities Uses administrative time effectively. Precepts visiting medical, nurse practitioner, and physician's assistants students and residents well, mentoring, encouraging, teaching, modeling. Provides appropriate supervision for nurse practitioners and physician assistants, reviewing and signing charts and fee tickets, teaching, being available as needed. Participates in staff devotions daily. Leads devotions periodically. Contributes to spiritual life of staff. Accountable to and active in a Bible-teaching church congregation. Suggests improvements, and works to help implement practice improvements. Assists with Esperanza promotional and fund-raising materials and events (optional). To carry out its mission, it is of critical importance for Esperanza Health Center (EHC) to keep our patients, families, and workforce safe and healthy and to support the health of our global community.
