Work Shifts 8:00 A.M - 4:00 P.M Job Description PURPOSE OF POSITION: The Community Liaison Level I serve as an integral member of the multidisciplinary team and participates in and/or assisting the team to work with the counselor and medical staff to obtain concrete community services including housing; obtaining medical insurance; obtaining entitlements; obtaining documentation for applying for immigration. The SUD populations are transient requiring support and guidance through particularly through early recovery. Often instability in the persons housing, finance, insurance and immigration status lead to resignation in the person sabotaging their efforts toward recovery. Hence the Community Liaison provides support and motivation for the patient to continue their SUD treatment. SUMMARY OF DUTIES AND RESPONSIBILITIES: • Communicating with internal (hospital clinics) and external services (i.e. vocational agencies, housing agencies, LEGAL SERVICES) educating the patients to secure services. This also provides patients a role model to effectively advocate for themselves (too often patients attempt to advocate for themselves using unproductive strategies. • To assist patients with social needs like food insecurity, housing issues, legal needs, insurance or other health. • Access and update specialized information systems to support the coordination of resources. • Assist patients with obtaining vital records to enable access to appropriate services. • Educate patients on available community services, health services, housing, vocational, legal services and patient rights; provide feedback from the patient. • Collect and track data to support achievement of patient-centered care plan using assessment tools, surveys. • Document each patient encounter in the electronic medical record (EPIC) and use other electronic systems, correspondences and other documents to the team of support community updates, maintaining confidentiality of documents complaint with 42 CFR. Minimum Qualifications 1. One year of full-time experience in a government agency or community organization in providing community services to the public or in assisting members of the community in obtaining services; or, 2. Completion of an approved six-month training program of combined classroom training and on-the-job experience in community liaison work, plus six months of full-time experience as described in (1).
