Palliative Care NP

Overview HarmonyCares is hiring a Palliative Care Nurse Practitioner (NP) to provide comprehensive, quality palliative care services to patients in collaboration with our team. This role involves patient visits, coordination of care, and independent decision-making under the supervision of the Palliative Care Medical Director. Responsibilities Provide medical expertise on pain, symptom management, and evaluation of terminal prognosis Coordinate care based on established palliative care guidelines and standards Collaborate with referring providers and internal partners Perform patient visits in various settings Oversight of pharmacy formulary and medication expenditures Collaborate with medical and operational leadership Qualifications Active and unrestricted state licensure as an Advanced Practice Nurse Enrollment with Medicare or state Medicaid 3 years of home-based or hospice care experience Strong interpersonal, communication, and clinical skills Proficiency in electronic medical records and clinical procedures Ability to work collaboratively and provide on-call coverage Salary USD $61,759.62 - $123,432.44 per year.
