We are looking for an OT to provide supervision and regular hours. The Occupational Therapist (OT) is responsible for assessing, planning, and providing rehabilitation services to patients or residents in a healthcare, long-term care, or assisted living facility. The OT focuses on helping individuals develop, recover, or maintain daily living and working skills, enhancing their independence and quality of life. This role involves working closely with patients, their families, and the interdisciplinary care team to create and implement personalized treatment plans that meet each patient's specific needs. Key Responsibilities: Patient Assessment: Conduct comprehensive assessments of patients functional abilities, including physical, cognitive, and emotional capacities. Evaluate patients' needs for occupational therapy services based on medical diagnoses, personal goals, and current functional status. Collaborate with physicians, nurses, and other healthcare professionals to ensure a holistic approach to patient care. Treatment Planning and Implementation: Develop individualized treatment plans based on patient assessments, setting realistic and measurable goals to improve daily living activities and occupational performance. Implement therapy programs that include exercises, activities, adaptive equipment, and environmental modifications to enhance independence in daily tasks. Monitor and document patient progress, making necessary adjustments to treatment plans to maximize outcomes. Therapeutic Interventions: Provide therapeutic interventions to help patients improve their ability to perform daily tasks, such as dressing, grooming, eating, and mobility. Teach patients and caregivers how to use adaptive equipment, such as wheelchairs, splints, and assistive devices, to improve safety and independence. Conduct therapy sessions in individual or group settings, adapting activities to meet the needs and abilities of patients. Patient and Family Education: Educate patients and their families about treatment plans, goals, and strategies for improving functional abilities at home and in the community. Provide guidance and support to caregivers to ensure they are able to assist patients effectively with their daily living activities. Offer recommendations for home modifications, adaptive equipment, and strategies to promote safety and independence. Documentation and Compliance: Maintain accurate and up-to-date documentation of patient assessments, treatment plans, progress notes, and outcomes in the electronic health record (EHR) system. Ensure compliance with federal, state, and facility regulations regarding occupational therapy services and patient care. Participate in care plan meetings, interdisciplinary team discussions, and quality improvement initiatives. Collaboration and Communication: Work closely with other healthcare professionals, including physical therapists, speech therapists, and nursing staff, to provide comprehensive care. Participate in discharge planning, providing recommendations for continued occupational therapy services or home care as needed. Communicate effectively with patients, families, and the care team to ensure that treatment goals are aligned with patient needs and preferences. Education: Bachelors or Masters degree in Occupational Therapy from an accredited program. Licensure: Current and active Occupational Therapist license. Experience: Previous experience in a healthcare, long-term care, or rehabilitation setting is preferred but not required. New graduates are welcome to apply. Skills: Strong clinical and patient care skills with the ability to develop and implement effective treatment plans. Excellent communication and interpersonal skills, with a compassionate and patient-centered approach to care. Ability to work effectively as part of an interdisciplinary team and collaborate with other healthcare professionals. Proficiency in electronic health records (EHR) systems and therapy documentation. Knowledge of adaptive equipment and therapeutic techniques for rehabilitation.
