This position covers two weeks per month to include Saturday, Sunday and Monday-Friday 5:00pm-9:00am. The rate of pay is $450.00 per week on-call and $34.00 per hour if in-house. This position is located at St. John's Meadows Hawthorne Assisted Living. Position Summary: The Hawthorne RN on-call will be on call at all times when an RN Case Manger is not in the building. The RN on-call may also be asked to provide on-call coverage during the RN case manager's sick time and vacations. The RN on-call will provide RN support to staff in the Hawthorne building during the non-working hours of the RN Case Manager. Responsibilities include: The RN on-call is responsible for responding to all calls within no more than 10 minutes. The RN on-call will ask sufficient, ample and appropriate questions of the person calling in order to provide the staff person appropriate direction. This information will also be sufficient, ample and appropriate to report and communicate with hospital staff if it is required. The RN on-call will use the information gathered from the conversation with the Hawthorne staff person to call and notify the elders POA/next of kin and/or emergency contact. The RN on-call may also be required to be in contact with 911, the elders physicians, Companion and Licensed Nursing support staff agencies, the elders families/POA, as well as the Hawthorne staff in the building, St. John's Meadows security staff, the RN Case Manager and the Hawthorne Program Coordinator. The RN on-call will come on site to the Hawthorne building and receive report from the Hawthorne nursing team no less than once weekly prior to starting his or her assigned working hours. The RN on-call will also use this time to read any notes left for the RN on-call by Hawthorne staff and will pick up any materials from the nursing office that have been left for the RN on-call. The RN on-call will review the materials in the RN on-call tote/box and collect any needed materials weekly from the nursing office (progress notes papers, elder lists, elder emergency phone lists, staff phone lists). It will be the responsibility of the RN on-call to have contemporary information for all Hawthorne elders including the details of the elders present care plans, their care needs, and any emergency contact information. If needed the RN on-call will be required to physically report to St. John's Meadows: Hawthorne. The need to report on site may be due to staffing shortage, a staffing call in that leaves the staffing ratio below minimum required for elder safety, natural disaster, the need for emergency evacuation, or in the event of any other unforeseeable emergency that occurs during the hours of duty of the RN on-call. The RN on-call will document the actions taken on all calls received during the RN on-calls working hours. The RN on-call will attend all mandatory trainings, in-services and meetings. The RN on-call will contact the Hawthorne Program Coordinator for consultation as needed. Qualifications: A RN license in good standing CPR certification Excellent customer service skills Physical Requirements: Standing, walking, sitting, climbing stairs, lifting 50 pounds or more, able to assist elders with their daily needs.
