Description NORTH END WATERFRONT HEALTH Department: Adult Medicine, Pediatrics & Family Medicine GENERAL SUMMARY/ OVERVIEW STATEMENT: The Population Health Management (PHM) Program at NEW Health leverages internal and external data to advance safe, high-quality, value-based patient care. Working closely with NEW Health leadership and community stakeholders, the Population Health Manager is responsible for designing, implementing, managing, and evaluating all PHM Program initiatives organization-wide including the community health worker program. Responsibilities include determining project needs and scope, planning and coordinating timelines and milestones with stakeholders, tracking deliverables, and measuring project outcomes and effectiveness. The Population Health Manager has a high degree of skill in data analysis, project management, and stakeholder engagement. This individual will support multiple efforts in a fast-paced, cross-functional work environment, coordinate workstreams across multiple teams throughout NEW Health, and continuously integrate process improvement into daily work. PRINCIPAL DUTIES AND RESPONSIBILITIES: I ndicate key areas of responsibility, major job duties, special projects and key objectives for this position. These items should be evaluated throughout the year and included in the written annual evaluation. Key Areas of General Responsibilities * Develop a keen understanding of primary care practice requirements for optimal, coordinated population health * Work as an effective team member of the population health management program * Support practice staff to develop creative processes to proactively manage target populations * Provide data management, coordination, and patient outreach as needed for specific target patient populations * Work collaboratively with co-workers to effectively resolve issues that impact departmental operations Key Areas of Quality Responisibilities * Co-Chairs the Quality Program and is responsible for leading performance management to ensure quality program attains annual targets * Oversees regulatory requirements for quality including HRSA, UDS, Joint Commission, C3, HEDIS, NCQA, CMS, Department of Health, etc. * Serves as a subject matter expert on quality measurement and improvement * Develops annual quality plan; monitors annual performance on quality goals * Leads participation within the C3 MassHealth ACO value-based care and sub-capitation program, including quality and care management activities Leads monthly quality working group meetings and bi-monthly senior leadership quality council meetings Supports the board of director's quality committee, attends quarterly meetings Works with the Diversity, Equity, Inclusion (DEI) committee to support health equity Develops and maintains quality program documentation, including policies and procedures, annual operating plans, and quality improvement work plans Performs performance reporting and analysis to inform leadership and drive decision-making Leads programmatic corrections when adequate progress towards goals is not being achieved Facilitates continuous improvement with applicable stakeholders to incorporate enhancements into programs as necessary to drive performance and efficiency Responsible for operationalizing new quality gap closure efforts and growth initiatives Collaborates with physicians, clinical personnel, and administrative leadership to remove barriers to success and achieve organizational goals Key Areas of Operational Responisibilities * Reporting to the Chief Operating Officer, leads the PHM Program and supports other programs and grants as assigned Coordinates and assists in grant projects and data management Tracks resources and expenses for each program or grant assigned Coordinates metric gathering and reporting for ongoing awards, reviewing new grants for reporting requirements Coordinates various meetings and program events (working groups, focus groups, trainings, etc.), both on-site and off-site, in collaboration with leaders and community partners Collaborates with leaders to prepare reports and presentation materials for meetings Attends meetings to take minutes, record, and track the status of deliverables Provides post-meeting support to ensure action items meet established timelines, goals, and milestones Monitors and adjusts activities to ensure milestones and deliverables are met Provides feedback on processes and procedures to management and others for operational improvement and efficiency Oversight of Digital PHM Tools * Responsible for managing organization-wide strategy related to digital EHR tools and data management processes * Serve as the primary organizational support for Epic population health tools, data analytics, and business intelligence tools * Develop a keen understanding of primary care practice requirements for optimal, coordinated population health * Leverages knowledge of Epic population health tools to benefit the PHM program. This includes aspects of the management of care across populations including effectiveness, safety, patient-centered, value-based care, equity, efficiency, and timeliness * Work with stakeholders and technical experts to implement population health and analytics tools within Epic and other business intelligence platforms * Leads the technical transition of the Federal Family Planning Annual Report 2.0 and the UDS+ modernization projects in Epic Qualifications QUALIFICATIONS: * Bachelor's degree required, master's degree preferred in Public Health, health Policy, Healthcare Informatics, or related field * Minimum of 2-5 years experience in a healthcare environment, strategy, or program design and evaluation * Experience in quality operations development and execution including project management and people management * Strong PC skills/Microsoft applications including Word, PowerPoint, Teams required * Strong analytic skills in Excel required; SQL or Snowflake experience a plus. * Experience with Epic Healthy Planet/Compass Rose, Tableau, or Arcadia PHM Analytics is a plus * Experience managing multiple concurrent large projects and project teams * Knowledge of Population Health Management or prior successful implementation of clinical quality improvement interventions * Strong analytic background and works with analytic and informatics partners to track and trend performance, create feedback mechanisms to physicians and operational teams, and optimize electronic environments to support quality efforts SKILLS/ ABILITIES/ COMPETENCIES REQUIRED: * Demonstrated ability to work independently and possesses the ability to learn quickly * Superior analytic and problem-solving skills with a high value in data integrity/accuracy * Ability to work cooperatively with other members of the team and staff, ability to handle frequent interruptions and adapt to changes in workload and work schedule; to set priorities and solve problems; to respond quickly to emergency requests. * Strong systems-level thinking and problem-solving skills * Strong interpersonal skills to elicit cooperation from a wide variety of sources, including upper management, other departments/teams, and community partners * Ability to coach/teach others about complex quality measures * Ability to translate complex technical concepts to non-technical audiences * Strong program and project management skills * Experience, knowledge, and interest with DEI, anti-racism activities, and/or lived experience with racism is highly preferred WORKING CONDITIONS: Office-based environment EEO Statement Massachusetts General Hospital is an Affirmative Action Employer.
