JobSummary: Serve as the Health Equity Program Coordinator for CaroMont Health focusing on advancing the organizational strategic priorities that identify and reduce disparities in access, experience, and clinical outcomes for the populations that we serve. Serve as the subject matter expert for using quality improvement approaches to successfully implement health related social needs and health equity programs. Ensure adherence with all Health Equity regulatory requirements for all enterprise risk contracts. Align projects, processes, and resource planning with organizational and Service Line goals. Develop and manage relationships with external organizations, including social service organizations and state/ local agencies. Manage a portfolio of multiple concurrent projects/programs. Manage changing priorities, escalating issues, and reset priorities as needed to meet deadlines and goals. Collaborate with data analytics to identify and refine analyses for projects, developing analytic and quantitative frameworks to assess the feasibility of opportunities and evaluate program performance. Formulate recommendations for key stakeholders through development of exceptional deliverables, presentations, materials. Effectively facilitate cross-functional meetings to successfully collaborate and build consensus with multiple clinical and non-clinical departments including Service lines. Candidate will need to be able to independently represent CaroMont with external organizations, including state and local agencies including Board positions. Qualifications: RN from accredited school of nursing with current RN license to practice in NC (NC license or multi-state (compact) license). BSN required. Three to five years of Quality, Clinical Performance Improvement, Case Management, Care Navigation or quality related tasks preferred. Detail oriented with ability to collect and analyze data. Excellent oral and written communication abilities. Effective time management skills. Ability to facilitate change and affect positive results. Proficient with web tools, databases, and Microsoft Office. Working knowledge of current local, state, federal regulations, and Joint Commission requirements in relation to health equity and quality. Thorough ability to communicate with patients, families, staff, physicians, administration as well as the community. Expert, proven ability to manage and motivate people. EOE AA M/F/Vet/Disability
