Competencies: (6) * EthicsIntegrity: 25% * Internal Controls: 15% * Problem Solving: 15% * Planning & Organizing: 15% * Interpersonal Relations: 15% * Promoting Physical and Emotional Well-Being: 15% Duties and Responsibilities: * Act as case manager to assigned caseload. * Maintain accurate individual records and files and submit reports on progress. * Attend and facilitate IP and semi-annual meetings. * Ensure individual's medical, dental, dietary, psychological and all other habilitative service recommendations and treatments are followed through on as identified on the individual's Plan of Service. * Communicate with interdisciplinary team, including DDS, familyguardian, registered nurses and behaviorist. * Receive direct supervision from the Director of Residential Fee for Service Programs. * Record and maintain the following records in a timely, consistent and accurate manner: program data, medical and medication records, financial records as applicable, individual's case notes in recreational activities, and all other related records as required. * Complete all necessary billing documentation and submit in a timely manner. * Actively participate as a team member on the Kennedy Center team and interdisciplinary team. * Assist with the development and actively participate in the implementation of Individual Program Plans. * Develop appropriate recreational activities for each individual served. * Maintain contact with families andor guardians as assigned. * Knowledgeable of and ensure the protection of each individual's rights in accordance with agency's policies and procedures. * Transport individuals for medical examinations, recreational, and other appointments as necessary. * Perform various household chores as assigned. * Complete annually the required in-services as well as other in-services that are directly job related as specified in the agency's policies and procedures. * Schedule medical appointments for individuals as assigned. * Keep electronic medical records up to date; appointments, consults, medications, doctor's orders and follow up needs as assigned. * Communicate with VNA and Health Care Coordinators as assigned. * Other related duties and responsibilities as assigned. Qualifications: * High School Diploma. * Valid Driver's License and ability to possess a State of CT Public Service License. * Must maintain a satisfactory driving record. * You are required to be in possession of a car on site during your entire shift to transport yourself andor individuals served to community sites in all weather conditions and throughout all neighborhoods. * Depending upon assignment, must be able to work in a variety of settings, weather conditions and locations. * Must maintain PMT, CPR certification and eligible for DDS medication certification. * Knowledge and skills in Microsoft Office, Therap and the internet. * Work a flexible schedule that may include early mornings, evenings, weekends andor holidays. * Work independently; utilize good judgment and possess a strong sense of initiative. * Communicate clearly in oral and written form on a constant basis. * Access all areas of residential facilities. Physical Requirements: * Provide hands on personal care which may include toileting, feeding, cleaning and all other activities of daily living skills. * Ability to transfer individuals requiring personal hygiene care. * Have physical endurance to use physical crisis intervention techniques which may include restraining individuals. * Provide physical intervention which may include lifting, mobility, first aid, and safety evacuations. * Climb stairs and move weights up to 10 lbs. on a consistent basis and 50 lbs. on an occasional basis.
