Our client is seeking a Case Manager in Richmond, Virginia SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY Provides case management support for adults with intellectual disabilities living in community based settings. Will provide some direct care and other risk management functions according to the need presented. Supports Program Supervisors/Directors to enhance the quality of care through individual choice in accordance with the agency's mission. Performs duties with focus on consumer-centered services, consumer choices, discretion and attention to detail in accordance with the agency's mission CLIENT'S REQUIRED SKILLS & EXPERIENCE *Manages a case load of 7-10 individuals to complete quarterly reports, plan updates, falls risk plans/assessments, annual paperwork, support instructions, and assessment updates as needed. *Provides support at annual meetings, client care specific meetings, and supports the completion of root cause analysis for individuals on assigned caseloads. *Will participate in On Call responsibilities for assigned Director Team, and respond to individual care needs and or critical staffing issues as needed. *Professionally communicates and advocates for clients in accordance with agency's code of ethics and all applicable DHS and Human Rights compliance. Performs other related duties as assigned. *Bachelor's Degree in Human Services or field related to community services is preferred but not required *One or more years of directly related experience or any combination of training and experience which provides the required knowledge, skills, and abilities *4 or more years of working experience preferred if you do not have a bachelor's degree in Human Services *Valid Virginia Driver's license and acceptable Department of Motor Vehicles Driving record and dependable transportation *Able to move safely within the home environment, which may include stairs and multiple stories *Able to lift up to 50 pounds to assist individuals who have physical challenges with positioning, transferring or ambulating as needed DTG ADVANTAGES * Work with one of the nation's largest behavioral health referral agencies * Flexible schedule as a Contractor.
