Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Duchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently looking to fill a full time Family Planning Program Assistant based in Peekskill, NY. This is a hybrid position. The Family Planning Program Assistant reports directly to the VP of Nursing. This position will collaborate with the Family Planning Project Manager, the Medical Director of Women's Health, Nurse Leaders, quality and grants departments. The FP Program Assistant will work closely with the Family Planning Project Manager and the medical care team to ensure that activities related to patient care, documentation, and the support for staff are optimized for efficient and compliant delivery of high-quality patient care, women's health and family planning services. ESSENTIAL FUNCTIONS • Responsible for implementing family planning program activities as outlined by NYS Family Planning Program requirements and Sun River Health guidelines. • Prepare annual, quarterly, and monthly report/work plans, conduct meetings as required and ensure timely reporting. • Coordinate reportable STI follow up with medical providers, patients, and health department. • Provide technical assistance and training to members of the medical care team regarding family planning program and services. • Review Ahlers, EMR and practice management data to ensure quality services in accordance with national best practices and NYSFP guidelines. • Collaborate with FP Project manager and grant staff in obtaining funding, planning and evaluating grant funded activities and programs. • Participate along with the FP Project Manager in clinical conference calls, staff meetings and trainings, and assist in coordination or provision of continuing education for clinical or support staff. • Participate in completing documentation of required information per NYSFP guidelines. • Collaborate, coordinate and support women's health access to care and quality by providing data, reports and analysis, meeting management and other ad hoc duties to improve patient care. • Participating in outreach events to promote and educate on family planning services and resources. • Promotes a positive work culture and upholds policies and procedures. • Ability to create tables and simple displays of information into spreadsheet software. • Ability to enter and manage data into a centralized electronic health record. • Other duties as assigned. EDUCATION/EXPERIENCE: • Bachelors Degree in Healthcare or related field, Masters preferred but not required • Minimum of 5 years' experience working with vulnerable populations preferably in a Women's Health/Family Planning Program • Experience working in an ethnically, culturally, and racially diverse environment is preferred • EMR experience required Job Type: Full-time Pay rate: $38,000-45,000 annual Responsibilities Relation to Mission The mission of Sun River Health is to increase access to comprehensive primary and preventive health care and to improve the health status of our community, especially for the underserved and vulnerable.
