Job Summary We are seeking a compassionate and skilled Licensed Vocational Nurse for our Long-Term Care (LTC) facility. The ideal candidate will provide high-quality nursing care to residents requiring long-term support, ensuring their comfort and safety. This role requires strong clinical skills, the ability to handle diverse patient needs, and a commitment to patient education and support. Duties Monitor and assess the condition of residents in the LTC facility. Collaborate with physicians and other healthcare professionals to develop and implement comprehensive care plans. Administer medications, perform interventions, and manage resident care as necessary. Educate residents and their families about treatment processes, medications, and follow-up care. Maintain accurate and thorough patient records using electronic health record systems. Ensure compliance with infection control protocols and patient safety standards. Participate in quality improvement initiatives aimed at enhancing resident outcomes and workflow efficiency. Experience Minimum 2 years of experience in Long-Term Care or a similar setting. Experience with geriatric care and chronic disease management preferred. Requirements Active NH State LVN license. BLS certification required; ACLS certification preferred.
