As an LCSW, you will play a crucial role in providing comprehensive care and support to individuals and families facing various psychosocial challenges. Key Responsibilities: Conduct thorough biopsychosocial assessments and develop tailored treatment plans for clients Provide ongoing psychotherapeutic counseling and support to patients and their families Collaborate with multidisciplinary teams to ensure holistic care and attend regular rounds and meetings Advocate for patients' needs and coordinate with community agencies to secure necessary services Maintain accurate and timely documentation of all client interactions and treatment progress Participate in performance improvement projects and ongoing professional development Conduct home visits and community outreach as needed to support client care Implement universal screening protocols to identify potential abuse victims and follow mandated reporting procedures Qualifications: Valid New York State LCSW license required Strong clinical assessment and intervention skills Excellent interpersonal and communication abilities Ability to work effectively in a fast-paced, multidisciplinary environment Commitment to ethical practice and culturally competent care Flexibility to work as an essential employee during various circumstances Please note that the salary range and/or hourly rate range of $40.00 - $45.00 per hour is a good faith determination of potential base compensation offered to applicants at the time of this job advertisement and may be subject to modification in the future.
