Why Suncrest Suncrest Hospice is seeking a full time RN to work as a integral part of our on-call team! This position works Friday 4:30pm until Monday 8:00am. Applicants must have great clinical and communication skills. The On Call nurse is responsible for answering calls from patients, families and customers. The On Call nurse will make nursing visits for symptom management, deaths, and admissions as needed. Visits occur in patient homes including private residences, assisted living communities and skilled nursing facilities. This position must also be passionate about hospice care and be a great advocate for both hospice services and Suncrest Hospice. Qualifications Current LPN or RN licensure (in good standing) in the state(s) of practice Hospice expereince Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and aide leadership. Work closely with patients on a 1-1 level but get strong support from a patient care team. Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations. Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns) Ability to learn an electronic medical records system.
