Your responsibilities will include: Regularly assesses and reassesses the nursing needs of the Hospice patient. Competently implements care needed for comfort and palliation of symptoms Provides direct client care and works closely with case manager in implementing care. Performs admission procedure as per RVNAHealth policies using electronic health record in a timely manner with accurate documentation. Develops a plan of care consistent with the physician's plan of treatment and patient/ family wishes. Coordinates the implementation of the plan of care for patients residing in SNF, ALF, hospital contract bed or personal residence. Provides dietary counseling. Provides Hospice nursing services, treatments and preventive procedures. Initiates nursing procedures appropriate for the patient's Hospice care and safety. Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patient's physical or emotional condition. Teaches, supervises and counsels the Hospice patient, family members and facility staff about providing care for the client. Experience: Graduation from an accredited school of nursing; BSN preferred Currently licensed to practice as a Registered Nurse in the State of Connecticut. A minimum of two years of nursing experience, preferably in medical/surgical, oncology , Critical Care, ED or labor and delivery nursing. Home Health Care and/or Hospice experience preferred. Active patient contact within past three years, preferred. Ability to work independently, make accurate, and at times, quick judgements. Able to drive in all types of weather.
