Location: UCMC Department: Care Management Services Hours: Part-Time, 16 Hours/Week Shift: First Registered Nurse (RN) Case Management, Part-Time, WOW Shift, Care Management Services UC Health is hiring a part-time Registered Nurse (RN) Case Management for the Care Management Service's department for the Work on Weekends shift at University of Cincinnati Medical Center. The Case Manager utilizes advanced nursing skills and knowledge of resource management and fiscal responsibility to coordinate the clinical care for a designated patient population across the continuum of care. The responsibilities include but are not limited to clinical effectiveness, discharge planning, care coordination, and clinical resource management. The Case Manager interacts with Medical, Nursing and Ancillary department staff to facilitate quality based, cost effective patient outcomes, and ensure care is provided in the most appropriate setting. Education and Experience: Graduate from an accredited school of nursing; Bachelor of Science in Nursing, minimum required. Current OH RN License 1-3 yrs.
