Job Description TOUR 2 - 9:00 A.M - 5:00 P.M AREAS OF RESPONSIBILITIES Nutrition Office, Kitchen, Inpatient and Behavioral Health Units SUMMARY OF DUTIES AND RESPONSIBILITIES Interview patients/significant others to obtain nutrition history, review medical records data, and complete nutrition assessments, reassessments, and malnutrition assessments according to the Evidence-Based Nutrition Practice Guidelines. Documents in Electronic Medical Record (EMR) in the ADIME format utilizing the Nutrition Care Process. Provides medical nutrition therapy (MNT), including discharge instructions, to inpatients and/or families and document nutrition intervention in EMR. Considers cultural and religious food preferences, language barriers, physical disabilities and age group when performing assessment or providing MNT. Determines malnutrition according to hospital-approved criteria, documents and communicates to the physician as per policy. Responsible for all aspects of patient nutrition care and education. Consult with members of the medical team regarding the nutritional needs of patients. Participates and attends interdisciplinary team rounds. Visits patients during meal periods and observes patient diet tolerance, intake and initiate corrections of diet as/if needed. Conducts patients' tray evaluations. Participates in data collection for performance improvement. Performs and maintains performance improvement programs as assigned. Participates in special nutritional projects for patients when requested, including the development of patient education materials. Participates in clinical committees, attends meetings, trainings, continuing education and in-services as assigned. Assists in the development of education and training programs for professional and auxiliary staff. Assists with the implementation of departmental policies and procedures. Participates in training of dietetic interns and new clinical dietitians. Maintains patient confidentiality and discards Protected Health Information (PHI) as per policy. Uses CBORD to enter patient's food preferences and allergies when needed. Participates in downtime procedures during EMR or CBORD failure. Performs related tasks as assigned and assists others in the department as requested by appropriate personnel. Minimum Qualifications Qualification Requirements: For appointment to Assignment Level I 1. Baccalaureate degree from an accredited college or university, with a major in Dietetics or Foods and Nutrition in a program approved by the American Dietetic Association (ADA); or, 2. Registration or eligibility for registration by the Commission on Dietetic Registration (CDR) as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist. For appointment to Assignment Level II 1. Registration or eligibility for registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist. For appointment to Assignment Levels III and IV 1. Registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist. Special Note for Dietitians assigned to Post-Acute Care (Long-Term Care) facility: Under the Medicare and Medicaid Programs, Reform of Requirements for Long-Term Care Facilities, as outlined in Section 483.60 of Title 42 of the Code of Federal Regulations all newly-hired Dietitians, Levels I-IV assigned to a Post-Acute Care (Long-Term Care) facility, are required to have New York State Certification, issued by the New York State Education Department (NYSED), as a Dietitian-Nutritionist at the time of hire. Dietitians, Levels I-IV hired prior to November 28, 2016, assigned to a Post-Acute Care (Long-Term Care) facility are required to obtain New York State Certification, issued by the NYSED, as a Dietitian-Nutritionist on or before November 28, 2021.
