Key Responsibilities: Evaluate, diagnose, and manage acute and chronic illnesses, providing holistic patient care. Perform phlebotomy and other routine lab procedures, ensuring accurate collection and handling of specimens. Coordinate care with specialists, social services, and community resources to enhance patient outcomes. Deliver telehealth services as needed, expanding access to care for our patients. Participate in quality improvement initiatives and the development of innovative care models. Educate patients on managing their health conditions and empower them to achieve optimal wellness. Provide timely and accurate documentation in electronic health record (EHR) systems, ensuring confidentiality and compliance with regulations. Skills & Competencies: Expertise in clinical decision-making, patient assessment, and phlebotomy techniques. Strong interpersonal skills with the ability to build trust and rapport with patients. Ability to adapt to new healthcare technologies and participate in continuous improvement efforts. Flexibility to accommodate varying patient volumes and clinic needs, especially for per diem shifts. Experience and Skills Requirements: Education: Master's degree in Nursing with certification as a Nurse Practitioner or completion of an accredited Physician Assistant program. Licensure: Current Nurse Practitioner or Physician Assistant license in Pennsylvania. Experience: Minimum of 2 years in a clinical setting, with community health experience preferred. Phlebotomy experience is required. Proficiency: Comfortable using EHR systems and telehealth platforms. Commitment: Full-time practitioners should be available during regular clinic hours, while per diem staff must be open to covering a variety of shifts.
