Habitat Health was founded in 2023 to deliver fully integrated health and social care to older adults on Medicare or Medicaid who wish to remain living independently in their homes and communities. For these older adults, Habitat Health has the opportunity to significantly improve clinical outcomes, extend lives and preserve families - whether of kin or of choice.
