We have an Exciting opportunity for a Certified Nurse Practitioner - Family Practice. You will be responsible for acting as the patient's primary healthcare provider, providing quality primary care to patients of all ages in an outpatient clinic setting. General duties include conducting physicals, ordering tests, educating patients about preventive care and prescribed treatments, prescribing medications within prescriptive authority and addressing barriers to patient care. The Certified Nurse Practitioner is a member of the Patient Centered Medical Home (PCMH) Care Team. All members of the PCMH Care Team will work cooperatively with and interact with the patients and other team members to coordinate patient care and achieve the stated patient and PCMH Team goals and objectives. PCMH goals and objectives include increased access to patient care, high quality patient care, and patient self-management of care. This position is full-time, Monday - Friday, no evenings, weekends or holidays.
