The Ambulatory Renal Nurse Navigator in a private nephrology practice plays a vital role in guiding patients through their kidney care journey. This position involves direct patient interaction, coordination of care, and collaboration with healthcare providers to ensure that patients with chronic kidney disease (CKD), those nearing dialysis, and potential kidney transplant recipients receive comprehensive, personalized care. The Nurse Navigator serves as the primary point of contact for patients and their families, helping them navigate the complexities of kidney care within the private practice setting; and other duties as assigned. EDUCATION and/or EXPERIENCE Bachelor's Degree, associate's degree, or diploma in Nursing from an accredited institution Current Maryland licensure or eligibility to obtain Maryland licensure Excellent customer service and communication skills Two to three years clinical experience CPR certification is required Strong technical skills to operate highly specialized electronic equipment Strong working knowledge with computers, computer equipment, data collection platforms (such as Excel)
