Transitional Care LCSW / LMFT Case Manager

LCSW/LMFT Transitional Care Case Manager HealthCare Support is actively seeking a LCSW/LMFT Transitional Care Case Manager to fill an opening with a Managed Care Organization in Rancho Cucamonga, CA. Daily Responsibilities for LCSW/LMFT Transitional Care Case Manager: Provide services to all Medi-Cal Members transitioning to and from hospitals, institutions, other acute care facilities, and skilled nursing facilities to home or community-based settings Assist with coordination of care, provided resource linkages, and collaborate with other Team Members Shift: Monday to Friday, 8am to 5pm/ Work Location: 2-3 days Onsite in Rancho Cucamonga, CA and 2-3 days from home Required Qualifications for LCSW/LMFT Transitional Care Case Manager: Master of Social Work and active CA LCSW or LMFT License 3-5 years of behavioral health experience Experience in motivational interviewing and/or other evidenced-based communication strategies Preferred Qualifications for LCSW/LMFT Transitional Care Case Manager: MCO and/or Case Management experience Bilingual in Spanish Benefits for LCSW/LMFT Transitional Care Case Manager: Immediate enrollment in Health Insurance Dental Insurance Life Insurance Employee Assistance Program (EAP) Access to Investment Accounts Career and educational tools within our Ingenovis ACT (Advocacy) Program Pay Details: $43.87 - $43.87 an hour Interested in being considered?
