Sun River Heath is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a tull time Psychotherapist for its A31 Sidney Baer (NYC) location. Coordinates and oversees all aspects of psychosocial programming. Assists in planning, developing, organizing and evaluating psychosocial, as well as, concrete needs of program participants. Essential Duties and Responsibilities: Assists in planning, developing and evaluating the psychosocial needs and concrete needs of program clients Coordinates and oversees all aspects of psychosocial programming and services including support groups for caregivers and participants and coordinates this aspect of the program with the recreational and health component Ensures that admission and discharge procedures are consistent with individual needs Ensures completion of psychosocial assessments of clients within the mandated time frame Ensures that staff who have case management responsibilities document case notes as required Participates in the interdisciplinary team care planning process Carries a reduced case management caseload, as necessary Serves as a resource for all other program staff; develops and maintains resource file Leads or co-leads psychosocial groups, including support groups for family members, significant others and care givers Provides psychotherapy for a select caseload Performs interagency liaison work with other service agencies, as needed Maintains client case notes and statistical records; ensures documentation of progress notes Participates in Quality Assurance program to evaluate, monitor, identify problems and institute revisions to maintain quality of client care and provide services in accordance with current standards of professional practice Attends and participates in seminars and continuing education programs to expand knowledge of current trends in the field of specialty and in issues concerns of the HIV population Attends all mandatory in-service programs Minimum Requirement: Master's degree in Social Work Minimum Related Work Experience: 2 - 4 years NYS Licensed LMSW, LCSW or LMHC Salary Range: $65,000 - $100,000 annual depending on licensure and experience Responsibilities Relation to Mission The mission of Sun River Health is to increase access to comprehensive primary and preventive health care and to improve the health status of our community, especially for the underserved and vulnerable.
