Employment Type: Part time Shift: Day Shift Description: SOCIAL WORKER - Behavioral Health Specialty EDDY SENIORCARE Program of All Inclusive Care for the Elderly Per Diem - Flexible Schedule Available! Eddy SeniorCare is a program of all-inclusive care for the elderly, with two centers: one in Rotterdam and one in Latham. This is a unique model of care designed to help seniors avoid/delay nursing home placement and remain in the community for as long as possible. The program serves individuals 55 years of age and older in zip codes nearby both our centers. Eddy SeniorCare serves as both payer (we provide their Medicare and Medicaid insurance) and provider (offering a full range of services--from primary care providers, to home health care, transportation, adult day care, activities, social work, RN case management, nutrition, home delivered meals and medications and medical supplies, and much more!). This is a wonderful opportunity to work as part of a closely integrated team meeting both the physical and psychological needs of our participants. : Works as an integral part of an interdisciplinary team to provide mental health services (counseling, resources, and education). Provides person centered, trauma informed therapy, brief counseling, and care coordination to participants and their families. Opportunity for group work in a Day Center setting. Experience: Clinical experience working with individuals in a behavioral health setting. Experience in substance use disorders and work with the geriatric population is a plus. Excellent assessment skills required and ability to assess for safety planning purposes as related to direct patient care. Educational Requirements: Master's degree in social worker. LCSW required. CASAC preferred but not required. Apply today for more information! Pay Range: $31.50 - $45.23 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
