Senior Care Nurse Practitioner

TruHealth is seeking a Senior Care Nurse Practitioner with experience in primary care or internal medicine, preferably in geriatrics. This position offers the opportunity to improve outcomes for long-term care residents by collaborating with a team of providers and case managers. The role involves providing specialized, coordinated health care to plan members, prescribing medications, monitoring patient compliance, and working as part of an integrated care team. The ideal candidate should have a Masters degree or higher, APRN or PA license, and at least 3 years of experience in clinical nursing or rehab in geriatric populations. Key responsibilities include assessing patients' medical and mental health needs, ensuring regulatory compliance, and providing personalized care plans to manage conditions and address barriers to care. The Senior Care Nurse Practitioner will also work closely with facility partners and health plan leadership to implement the national model of care in compliance with all regulations. This role may involve providing training and advice to facility staff.
