Hospice Clinical Liaison Social Worker, RN or LPN

Reporting to the Director of Provider Relations, this position is responsible for proactively building solid relationships in select Boston academic hospitals and surrounding area to deepen and expand our referral base. This role will be responsible for coordinating and facilitating communication between Care Dimensions and targeted referral sources while also providing clinical patient assessment and ensuring continuity of care between all settings. Qualifications: LISW/LCSW Social Worker or LPN or Registered Nurse, currently licensed in Massachusetts in good standing - preferably one year experience in skilled/acute setting Excellent customer service, organization, problem-solving skills Ability to build and foster relationships Demonstrated ability to assess and respond to the needs of patients and families Educate health care professionals, patients and families in all setting regarding the benefits/appropriateness of hospice care Demonstrated ability to be self-directed, flexible and cooperative in fulfilling role obligation Ability to travel within assigned territory #INDRN The Pay Range listed is the anticipated hiring range of this position. Precise pay rates are determined through consideration of one or more of the following: education, training, relevant experience and work location. There are some cases in which the pay rate could exceed the range listed on this job post.
