Coordinate social services and case management functions into patient care processes Conduct concurrent medical record review and act as a patient advocate Promote effective resource utilization and evaluate patient satisfaction Facilitate interdisciplinary patient care rounds and conferences Assist in the development and implementation of patient care policies and protocols Education and experience criteria: Associate degree in Nursing required, Bachelor's Degree in nursing preferred Active Registered Nurse (RN) license in State of employment Minimum of 2 years' clinical work experience and 1 year of case management experience Certified Case Manager certification preferred
