Serve individuals who have been diagnosed with serious and persistent forms of mental illness, predominantly but not exclusively the schizophrenia spectrum disorders. Office-based role. Periodic instances when services are provided in the community Work collaboratively with the multidisciplinary team comprised of psychiatrist/advanced practice nurse, case managers, peer specialists, substance abuse specialists and nurses. Formulate and implement treatment plans. Provide counseling and clinical intervention with the goal of improved independent function.
