Job Details : (1) Licensed Clinical Social Worker - Humboldt Park Health - Chicago, IL *Black out dates : 12/15-01/15* • Contract Length: 13 weeks • Start Date: ASAP (bi-weekly orientation) • Guaranteed Hours Per week: 40 hours/wk (80 biweekly - ability to cancel 1-12 HR shift per 2 weeks) • Primary Location: Humboldt Park Health - Chicago, IL • Shift Requirements: 8 HR Days (8-430pm) • Weekend/Holiday: every other weekend/every other holiday rotation with staff • On Call/Call Back Requirements: Call coverage one Saturday per month • Required Certifications to be Attached upon Submittal: Master's Degree in Social Work (MSW), IL LCSW or LSW. CPI certification. • Minimum Experience: 2 years' experience in social services and counseling in a hospital acute care setting with a diverse population • Charting Experience Required: Meditech preferred, not required • Unit Information (Bed size, Case type, Patient ratio): Behavioral/Psych Units (3A up to 24 pts, 3B up to 15 pts, 3D up to 15 pts). Busy/fast paced environment. Currently, the unit is typically providing care for 40 patients. RNs, Mental Health Counselors, and CNAs also staffed on the unit. Ratios are typically 2 RNs to 3 MHC/LCSWs per section. • Hard worker, Resilient and Manages Change well, Social candidates • Scrub Colors: Business Attire • COVID Vaccine Requirements: COVID Vaccination with a booster is required unless a medical or religious declination is applicable. Declinations are approved by Humboldt. • GSA hours are not guaranteed the week of Orientation. Traveler is guaranteed set hours per week, except during holidays where unit is closed. 12 hours non-billable orientation.
