Job Title: Registered Nurse (RN) - Flexible Position for Group Home Programs Location: Norwalk, Darien, New Canaan, Wilton and Westport Position Type: Contract (Flexible Scheduling Available) About Us: Join our dedicated team at STAR, Inc., Lighting the Way, where we are committed to providing exceptional care and support in our group home programs. We are looking for a compassionate and skilled Registered Nurse (RN) to join us on a flexible contract basis. Role Overview: As an RN with us, you will play a crucial role in delivering high-quality nursing care to residents within our group home setting. Your expertise and dedication will ensure their health and well-being while working in a supportive and dynamic environment. What You'll Do: Individual Care: Develop nursing plans supporting health care needs of the individuals, write nursing reviews and monthly nursing notes. Medical Management: Administer medications and treatments as prescribed, keeping meticulous records and monitoring for any side effects or reactions. Provided oversight to the med administration staff, review documentation to ensure certified staff follow procedures for reporting medication errors. Health Monitoring: Monitor medical and dental needs, addressing any health issues promptly and coordinating with the healthcare team. Care Coordination: Collaborate with physicians, therapists, and other professionals to create and implement effective care plans, ensuring seamless coordination of care. Emergency Response: Act swiftly and effectively in medical emergencies, providing first aid and emergency interventions as needed, and liaising with emergency services when required. Education and Support: Educate Individual and staff about health conditions and treatment plans. Support and guide staff on health-related matters. Documentation: Maintain accurate and comprehensive medical records in compliance with regulatory standards and organizational policies. Compliance: Ensure adherence to healthcare regulations, standards, and best practices, maintaining a safe and compliant environment. What We're Looking For: Education: Valid RN license and current registration in CT. Associate's or Bachelor's degree in Nursing from an accredited institution. Experience: Experience in group home, long-term care, or similar settings preferred. Proven ability to provide excellent care in a residential setting. Skills: Strong clinical skills, with a focus on assessment, planning, and implementation of nursing care. Excellent communication, interpersonal, and organizational skills. Certifications: Current CPR and First Aid certification required. Requirements: Ability to work independently and as part of a team. Flexible schedule to adapt to varying needs and individual requirements. Reliable transportation needed.
