Registered Dietitian- Livonia-20 Hours/Week Day Shift

Unit: St. Mary Dialysis ( CA/CC & HHD) St. Mary Home Dialysis is a unit attached to an outpatient dialysis facility . The facility oversees peritoneal and home dialysis patients. Census is 16 PD and 1HHD patient. Staffed with full-time Nephrologists. In addition, we have a part-time Home Nurses and a travel pool home dialysis technician. In addition, the unit has multiple support staff members that are shared with the outpatient clinic including front desk team and tech leader staff ( machine maintenance) that may assist with operational duties within the clinic. We case manage 17 pts who are monitored by connected health platform for Baxter and NxStage systems. Patients are trained to perform dialysis treatments at home within the clinic. PD training takes 7-10 days and HHD training takes 4-6 weeks. The home team schedules monthly clinic visits for all home patients with the interdisciplinary team and lab draws for PD patients. Our team provides pt education as well as referrals to transplant services for sites the pt requests to be referred to. The unit is typically open Monday through Friday 8am to 430pm for pt care hours with some flexibility based on the needs of the clinic. Home Nurses are scheduled to take daily after hour on-call including weekends. The home clinic is closed most weekends and holidays depending on the needs of the clinic ( training or clinical need for a patient ). Responsible for all aspects of nutritional needs of dialysis patients. Will strive to fulfill the organization's mission, vision, values, philosophy and goals. Works closely with the interdisciplinary team members in order to care for the dialysis patient. Develops a close working relationship with other dietitians in order to facilitate information flow and educational efforts. Reports to designated supervisor. EDUCATION AND EXPERIENCE: Bachelor of Science in Nutrition. Completion of an approved dietetic internship. ADA Registered and minimum of 1 year professional experience as RD in clinical nutrition.
