Responsibilities Wellstone Regional Hospital is a 100-bed acute care facility located in Jeffersonville, Indiana and has been providing quality health care to the residents of Southern Indiana and the Louisville area since 2003. Wellstone specializes in the treatment of Adolescents, Children, Adults, and the CD population. In addition to our in-patient services, we offer out-patient programs as well. Wellstone is currently searching for a full-time Discharge Planner for our Social Services Department. The Discharge Planner is responsible for the majority of discharges on units within WRH. The Discharge Planner completes individualized discharge and safety plans and provides patients with a review of treatment options beyond WRH. The ideal person represents WRH in coordination of treatment follow up for patients within the community. The Discharge Planner works closely with other members of the social services team from the time of intake to plan for an appropriate discharge plan. This position is a Monday-Friday, 8a-4:30p hourly position. Job Duties: Completes the majority of discharge plans on patients admitted and discharged from WRH Acute Services, as per policy Provides and documents discharge planning and crisis/relapse prevention planning which occurs during the discharge process Identifies a wide array of social service and treatment resources within the communities served by WRH Completes comprehensive reviews of follow up treatment with discharging patients to ensure smooth transition from the facility to the community. Attends interdisciplinary team and social service team meetings as necessary Makes and maintains contacts within communities served by WRH Provides outreach to community agencies on behalf of patients when necessary to facilitate a smooth transition to the community Completes outcomes measurement tools per policy Performs other duties as assigned/required by this position. Qualifications Requirements: Bachelor's Degree from an accredited college or university; Prefer atleast 1 year expereince in a psychiatric or mental health setting desirable; Service Excellence mentality; Some knowledge of medical terminology; Ability to react calmly and effectively in emergency situations; Organizing and prioritizing workloads to meet deadlines; CPI and CPR; Ability to communicate effectively, oral and written; Professional, exercises good judgement.
