CareATC is looking for an empathetic and driven RN Care Coordinator for a part-time opportunity at PPL's Healthy for Life clinics providing education, support, and coordination of services for the diabetic population within the service territories of KY, RI, and PA. Certified Diabetes Care and Education Specialist (CDCES) is strongly preferred. Must have experience with Diabetes Management and Education. This opportunity will be 20 hours per week, working ideally 3-4 days. This is a hybrid role requiring onsite services in Louisville, KY. Must live in the KY area. About This Opportunity: CareATC is looking for an experienced Registered Nurse with strong communication skills to collaborate with our health center team to provide platinum-level patient care and excellent customer service. The RN Care Coordinator is a central member of the team, providing clinical oversight and management of Chronic Disease Management initiatives. They provide one-on-one advocacy, chronic disease management, case management, and care coordination, and assist in treatment plan implementation. The RN Care Coordinator is responsible for conducting outreach and enrollment of identified patients participating in Chronic Disease Management initiatives. The RN Care Coordinator collaborates with all members of the Health Center team to provide clinical guidance, education, and program oversight. They work closely with physicians, health center staff, ancillary providers, and other members of the Total Health Solutions team coordinating patient care and ensuring team communication and collaboration. Hours: 20 hours per week Schedule: Flexible within Clinic Operation Hours. Ideally 3-4 days per week Remote Type: Hybrid The role of the RN Care Coordinator: Clinically assesses patients enrolled in Diabetes Management Program initiatives. Must have knowledge and experience in interpreting lab values and clinical measures in completing an accurate patient assessment. Experience in motivational interviewing preferred. Efficiently and accurately charts in the EMR system for patient interactions and care plans. Tracks and audits charts for all enrolled patients. Provides updates to management in a timely fashion, when requested. Leads, organizes, and documents interdisciplinary Diabetes Management team meetings with assigned Health Center staff and ancillary service providers. Strong organizational, communication, flexibility, and time management skills are required. Conducts clinical outreach to multiple patients that utilize Chronic Disease Management services. Empowers patients to self-manage their health and chronic conditions. Provides clinical support, as needed to all members of the Health center team and ancillary service providers participating in Chronic Disease Management initiatives. Effectively collaborates and integrates with Directors of Operations, Health Center Managers and other members of the Clinical Innovations team. Utilizes various technology platforms to identify high-risk populations for outreach. Stays up to date on clinical best practices to provide relevant and effective clinical education programming. Conducts large telephonic outreaches to inform patients of new programs and opportunities. Assist with creating educational handouts for patients/company Assists in Referral management for patients enrolled in Diabetes Management program Other duties as assigned. Qualifications Education: Bachelor's degree in Nursing from an accredited university. Experience: Registered Nurse, Multi-state license required. License must be current and in good standing. Minimum 5 years of working in healthcare setting or similar setting. Working knowledge of health insurance plans, electronic medical records, and Microsoft Office platforms. Must possess a current CPR certification and current health records with the appropriate immunizations to work in the health care field (hepatitis B and tuberculosis). Must be able to work independently, exhibit critical thinking, and prioritize tasks to meet commitments that are aligned with department goals. Strategic thinker that can analyze problems, prioritize solutions, and manage multiple complex situations, while collaborating with team members as required. Strong communication skills.
