Qualifications Required and Preferred: Associate's Degree in Nursing OR 3-5 years of nursing experience, preferably in home health, ambulatory care, community, public health, MLTC, coordinating care across multiple settings and with multiple providers - - Required Current Registered Nurse license without restrictions must be able to qualify for AK & WA RN licensure - Required Experience in maintaining a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature - Required Must be able to work Pacific Coast time during standard business hours - Required CCMC (Commission for Case Manager Certification) - a plus - Preferred Knowledge and experience teaching self-management. - Preferred Exceptional communication skills, both written and oral, ability to positively influence others with respect and compassion; fluency in a foreign language is a plus.- Preferred Strong work ethic built on a foundation of proactivity and teamwork. - Preferred Ability to navigate ambiguity with the aid of structured problem-solving techniques.- Preferred Committed to the practice of inquiry and listening. - Preferred Experience with electronic health records & Microsoft Office- Preferred RN licensure in Alaska and Washington is preferred (if not, Reimbursement is available)- Preferred Technical Requirements: We require that all employees have the following technical capability at their home: High speed internet over 10 Mbps and, specifically for all call center employees, the ability to plug in directly to the home internet router. This position is also eligible for a bonus component that would be dependent on pre-defined performance factors.
