Responsibilities The Palliative Care Nurse Practitioner (PCNP) works in collaboration with HarmonyCares staff to provide comprehensive, quality palliative care services to patients under the direction of the supervising Palliative Care Medical Director. The PCNP will be responsible for providing patient visits for palliative care patients, both new and established and coordinating their plan of care with other designated staff, especially the palliative care team. Patient visits will include visits to patient's homes, facilities and hospitals. In addition, the PCNP will engage in independent decision-making about a patient's healthcare concerns and provide palliative care services on an as needed basis. Palliative Care Directors will be available for consultation, collaboration, or referral. Essential Duties and Responsibilities The main function of the PCNP will be to provide medical support and coordination to palliative care patients and family members. This includes such duties as: Provides medical expertise on pain, symptom management, and the evaluation of terminal prognosis to patient, family members and staff: Ordering of appropriate therapies and tests when needed Seeing on average a total of 6-8 patient visits per day Ensure that the care being provided is based on established palliative care guidelines and standards Regularly contacts and collaborates with HarmonyCares Medical Group (or other referring providers) regarding new patient referrals and existing patients Interacts with attending physicians, as necessary, regarding the care and treatment of patients in the Palliative Care program and works collaboratively with internal partners Performs patient visits primarily in private residences, long-term care facilities, assisted living facilities, hospital or other locations Provides expertise and oversight of pharmacy formulary and medication expenditures Collaborate with Chief Medical Officer and Regional Director - Operations, Palliative Care Performs other duties as assigned by Operations Qualifications Required Knowledge, Skills and Experience Advanced Practice Nurse with appropriate active and unrestricted state licensure Valid NPI#, DEA and CTP Enrolled with Medicare or state Medicaid or eligible for enrollment 3 years of home based or hospice care or relevant experience Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community Knowledge of related accreditation and certification requirements Knowledge of the interdisciplinary team concept Knowledge of patient electronic medical record and obtaining patient histories Knowledge of health education theory and practice Knowledge of clinical operations and procedures Knowledge of current principles, methods, and procedures for the delivery of medical evaluation, diagnosis and treatment in palliative care Ability to work collaboratively with referring, primary, consulting physicians and other members of the interdisciplinary team to provide appropriate palliative care to patients Ability to work with other health care providers in long term care facility settings Ability to provide on-call coverage in an equal rotation Ability to maintain quality, safety, and/or infection control standards Demonstrates excellent observation, verbal and written communication, problem solving and organizational skills Demonstrates effective leadership styles to foster a positive work environment Proficient in personal computer use including email, clinical, word processing, and spreadsheet software etc Posted Min Pay Rate USD $61,759.62/Yr.
